Provisioning form

This form is used to request services for an employee or contractor. 

1. You must be logged into Our People to use this form properly.
2. The new or existing person must have a computer account and email address before requests can be made.
3. If this person will have an office at RRU Colwood or RRU Langford, please connect with Space Planning via the Space Request form before continuing. Come back to this form once you've been assigned space for the new person.
